Juniper Health collects and organizes the medical and health information you need quickly and easily in a way that makes sense. Juniper Health provides a customized set of medical recommendations and health guidelines based on your medical data, health profile, and personal preferences all in a secure and professional environment.
Juniper Health understands that your health is a very personal matter, and we appreciate the trust you place in us to treat your information carefully and sensibly. We have taken precautionary measures to make all information received from you secure against unauthorized access and use. Security measures are in place to protect against the loss, misuse, or alteration of information that we have collected from you at our Site.
We collect personal information when you voluntarily provide it to us or authorize us to collect it on your behalf. The types of personal information collected may include your full name, gender, email address, zip-code, age and your picture profile. Some of the information is non-mandatory and it is up to you whether or not you choose to share it. We may receive and store any information you enter on Juniper Health or provide to us in any other way. The personal information you provide may be used for attribution to comments you post to the Site, to operate and improve the Site and to better deliver our services.
Creating an Account and Records
The first time you visit Juniper Health you will be asked to create an account. To create an account, you must provide personal information, such as your email address; you will also be asked to create a password. To better assess and provide you with customized recommendations, Juniper Health will request additional information, such as your name, gender, ethnicity, height, weight, and age.
An account allows you to utilize the health care related tools offered on Juniper Health. Once you have an account, you can create health records for yourself and your family members (to the extent you have the right to provide their data). When you create a health record, you become a custodian of that record. You choose what information to put in your records.
If you choose to create a profile for a family member we will ask for their name, gender, ethnicity, height, weight and age as well. This information is used for the same purpose as your own personal and health information. Please see “How We Use Your Personal Information” for how we will use your family member’s information.
Whether you are completing a profile (Personal Health Record) for yourself or others in your family, you will be led through a comprehensive interview giving you the opportunity to record personal health information on a wide array of subjects including: lifestyle and habits, general health, family health history, surgeries and procedures, allergies, medications, infections, vaccinations, eyes/ears/nose/throat, heart and cardiovascular system, lungs and respiratory system, digestive system, kidney and urinary system, endocrine system, reproductive and sexual health, skin, muscles and bones, blood and lymph system, nervous system, mental health, vital statistics including blood pressure, weight, blood sugar, etc.
Once you register to become a member of Juniper Health you will have the opportunity to create your own personal profile. Your profile will allow you to provide additional personal information that is not required for registration, for example: your interests, activities, and personal story. You can change or update your profile information, including your privacy preferences at any time.
When you send email or other communications to us, we may retain those communications in order to process your inquiries, respond to your requests, and improve our services.
You can review and update your account information or your family members and modify, add, or delete any optional information by signing into your Juniper Health account and editing your account profile.
You are the custodian of any records you create on Juniper Health. You can close your account at any time by signing into your Juniper Health account and editing your account profile.
How We Use Your Personal Information
We use your personal information collected on Juniper Health to tailor your Juniper Health experience, show you the content in which we think you might be interested, and display content according to your preferences. We do not use or disclose your personal information except as described in this Privacy Statement.
We use your personal information to provide you with important information about Juniper Health, including critical updates and notifications; to modify your Juniper Health plan to better address your health needs and progress; and to determine appropriate services relevant to you, your lifestyle and your geographic location.
We may send email communications to you related to general health benefits, Juniper Health updates, health conditions and/or recommendations, and general health topics. These emails are required as part of the service and are not promotional in nature.
We may release personal information to third parties: (1) as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, a legal process served on our Web site, valid legal requirements such as a statute, regulation, search warrant, subpoena or court order; or (2) in special cases, such as a physical threat to you or others, a threat to homeland security, a threat to our system or network, or cases in which we believe it is reasonably necessary to investigate or prevent harm, fraud, abuse, or illegal conduct. In the event that we are legally compelled to disclose your personal information to a third party, we will make reasonable efforts to notify you unless doing so would violate the law or court order. Personal information collected on Juniper Health is currently stored and processed in the United States.
We use other third parties such as an email service provider to send out emails on our behalf. When you sign up for blog communications, we will share your name and email address as necessary for the third party to provide that service.
These third parties are prohibited from using your personally identifiable information for promotional purposes.
How We Use De-identified and Aggregated Information and Statistics
We may use de-identified and/or aggregated information gathered from Juniper Health and related statistics to improve the services provided through Juniper Health, and for certain research and analysis including clinical medical research. We may also provide this information to our partners for such purposes. De-identified information does not include personally identifiable information such as name, email address, or address, but may include gender, height, weight, postal code, and medical information. Aggregated information is information from multiple users that is not associated with any individual user account.
De-identified and/or aggregated information does not allow you to be contacted nor does it disclose your personal identity.
Security of Your Personal Information
We are committed to protecting the security of your personal information. We use a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, and disclosure. For example, we store the personal information you provide encrypted on computer servers with limited access that are located in controlled facilities. We restrict access to personal information to Juniper Health employees, contractors, and agents who need to know that information in order to operate, develop, or improve our services. These individuals are bound by confidentiality obligations and may be subject to discipline, including termination and criminal prosecution, if they fail to meet these obligations.
The security of your personal information is important to us. When you enter sensitive information (such as personal health information) within your account profile, we encrypt that information using secure socket layer technology (SSL).
Your Internet browser can be set to reject all cookies or to accept cookies from only one specific site, however If you reject our cookies you may not be able to access certain areas and functionalities of Juniper Health.
