Juniper Health collects and organizes the medical and health information you need quickly and easily in a way that makes sense. Juniper Health provides a customized set of medical recommendations and health guidelines based on your medical data, health profile, and personal preferences all in a secure and professional environment.

Creating an Account and Records

The first time you visit Juniper Health you will be asked to create an account. An account allows you to utilize the health care related tools offered on Juniper Health. Once you have an account, you can create health records for yourself and your family members (to the extent you have the right to provide their data). When you create a health record, you become a custodian of that record. You choose what information to put in your records.

If you choose to create a profile for a family member we will ask for their name, gender, ethnicity, height, weight and age as well. This information is used for the same purpose as your own personal and health information. Please see “How We Use Your Personal Information” for how we will use your family member’s information.

Whether you are completing a profile (Personal Health Record) for yourself or others in your family, you will be led through a comprehensive interview giving you the opportunity to record personal health information on a wide array of subjects including: lifestyle and habits, general health, family health history, surgeries and procedures, allergies, medications, infections, vaccinations, eyes/ears/nose/throat, heart and cardiovascular system, lungs and respiratory system, digestive system, kidney and urinary system, endocrine system, reproductive and sexual health, skin, muscles and bones, blood and lymph system, nervous system, mental health, vital statistics including blood pressure, weight, blood sugar, etc.

Once you register to become a member of Juniper Health you will have the opportunity to create your own personal profile. Your profile will allow you to provide additional personal information that is not required for registration, for example: your interests, activities, and personal story. You can change or update your profile information, including your privacy preferences at any time.

When you send email or other communications to us, we may retain those communications in order to process your inquiries, respond to your requests, and improve our services.

You can review and update your account information or your family members and modify, add, or delete any optional information by signing into your Juniper Health account and editing your account profile.

You are the custodian of any records you create on Juniper Health. You can close your account at any time by signing into your Juniper Health account and editing your account profile.

How We Use Your Personal Information

We use your personal information collected on Juniper Health to tailor your Juniper Health experience, show you the content in which we think you might be interested, and display content according to your preferences. We do not use or disclose your personal information except as described in this Privacy Statement.

We use your personal information to provide you with important information about Juniper Health, including critical updates and notifications; to modify your Juniper Health plan to better address your health needs and progress; and to determine appropriate services relevant to you, your lifestyle and your geographic location.
